2. Love the Pre-frame of the Call to Action, that is a GREAT tip! Here's another one: use the "Pause" annotation at the last second of your video and it will hold your call to action on screen without ending and displaying "related videos" that are from other channels!
3. Regarding keywords, I use Google Adwords and look for keywords that a minimum of 3-5,000 Exact Searches per month and have a low competition of videos when I type that keyword into Youtube. That's the trick, look for keywords that yield a low number of video results inside Youtube and it's far easier to rank higher.
